Udaipur News: Udaipur Collector Tarachand Meena took a special meeting of the concerned departmental officers on Monday to get the ex-gratia amount to be given to the relatives on death due to Kovid-19 from the state government. In this, instructions have been given to take action in the form of a campaign to get the assistance amount to all the eligible persons.
Collector gave instructions
Collector Meena said that the families of those families who have lost their members from Kovid will be given an ex-gratia amount of Rs 50 thousand from the government. He has given instructions to the officers. He said that in this regard, through the subordinate departmental personnel, call personally to the families of all the deceased and inform them to apply through e-mitra. Also, give information about the process of application.
On this basis, assistance will be given
CMHO Dr. Dinesh Kharadi told that for ex-gratia assistance, the applicant will have to keep in mind that the cause of death is recorded as Kovid-19 on the death certificate of those who died in the hospital after suffering from Kovid-19. Persons who have died within 30 days from the date of testing positive for COVID-19 or after being found clinically COVID-19. Even if such a death has happened outside the hospital. Such Kovid positive patients who have died during continuous hospitalization, even if they have come negative. The death certificate for COVID-19 will be issued by the hospital concerned if the death occurred in the hospital. In all other cases, the decision will be taken by the committee constituted at the level of District Collector.
These documents will be required
Dr. Kharadi told that the relatives of the deceased can apply free of cost through e-mitra along with documents. Documents required for application include application, identity card of the deceased (Aadhaar card or other identity card), death certificate (issued by Panchayat, Municipality, Municipal Corporation), relationship of the applicant with the deceased (essential certificate such as ration card , Aadhar Card etc.), Jan Aadhar Card of the applicant, Cancelled Check / copy of Bank Passbook of the applicant updated in Jan Aadhar Card, Death Certificate issued by the Hospital (Form No.-4).
Dr. Kharadi told that those whose applications have been sent back, they will have to send a new affidavit (which has been updated in the software today) by filling it back and filling other deficiencies.
Chief Minister Corona child support to the orphan child
Mandhata Singh, Assistant Director, Social Justice and Empowerment Department, said that the children orphaned due to Corona will be given Rs 1 lakh immediately as Chief Minister’s Corona Child Support. 2500 per month till each boy/girl completes the age of 18 years and one time assistance of Rs 5 lakh will be given on attaining the age of 18 years.
For this assistance, certificate of upbringing by the guardian of the orphan child / commendation letter, ration card / original residence / rent name (above 3 years), certificate of studying children / registration in Anganwadi center (educational status) for) or studied regularly in the previous session.
Affidavit of not benefiting from Corona Warriors, child’s age certificate (birth certificate etc.), joint account of child and parent, Jan Aadhar, death certificate (parents) and proof of death from Kovid in respect of medical and Verified list/certificate documents received from health department are essential.
Chief Minister Corona Widow Assistance Scheme
Assistant Director Singh said that in the Chief Minister’s Corona Widow Assistance Scheme, 1 lakh rupees will be given to the widowed woman from Corona. The widow woman will also get a pension of Rs 1500 per month for life. Documents required for this scheme include ration card/original residence/rent name (more than 03 years) affidavit of not being benefited in Corona Warriors scheme, bank account copy, Jan Aadhaar, death certificate (husband) proof of death from covid Verified list/certificates received from the Department of Medical and Health in this regard.
Chief Minister Corona Palanhar Assistance Scheme
Singh said that under the Chief Minister’s Corona Palanhar Sahayata Yojana, one thousand rupees will be given per month till the boy/girl of a widowed woman from Corona completes the age of 18 years. 2 thousand rupees per child / girl child per year will be given as a lump sum till the widow woman’s boy / girl completes the age of 18 years. For this, mother’s ration card / original residence / rental name (more than 03 years), affidavit of not being benefited in Corona Warriors scheme, bank account copy of widowed mother, Jan Aadhaar, death certificate (father), father’s covid Verified list/certificate received from the Medical and Health Department regarding the certification of death, certificate of children being studied/registration in Anganwadi center (for academic status) or studying regularly in the previous session and certificate of age of the child Documents (Birth Certificate etc.) will be required.
